INSTRUCTIONS:

Please fill out the Submission Form below, attach a photo of your military hero, and provide a copy of their DD-214/discharge papers (if available). Also include a check in the amount of $167 made payable to: “The Town of Greensboro Volunteer Fire Department”

If dropping off in-person, please do so at:
